Thank you for purchasing My Name Picture Book! The form below will allow you to easily customize the book with the child's name and up to 6 pictures that are meaningful to the child. If you would like to present a gift certificate so that pictures can be selected later, please download PDF file or e-mail us if you would like a nice, color document mailed to you.

After completing this form and uploading the files, the custom picture book will be shipped in approximately 1 week.

If a name other than a child's first name is used, enter the name of the book and there will be one page for each letter in the name.

Custom Pictures	Upload up to six pictures that you would like to be included in the book. Select the letter of the page the picture should appear on, then upload the picture. Please follow these guidelines for the pictures: - Please Include child's name or current date in the filename - Maximum file size = 500kb or 4"x 6" [how to change file size] - Acceptable file types: .jpg .bmp .gif .tif [how to change the file type]
